• Passport : Valid passport (with minimum validity of 18 months)
• Photo : A recent photograph
• Proof of Finance : Must be minimum (USD 2,000 or equivalent) in a 3 months period (including name, date of period, and balance account).
• Additional Documents Requirements :

A. Spouse of Indonesian Citizen:

1.  An application from your Indonesian spouse.
   - Evidence that the foreigner has lawfully married such as:
2. if the marriage is conducted outside the Indonesian Territory: Proof of reporting or registration at the Indonesian Mission or at the authorized agency in Indonesia, and a marriage certificate that has been translated into Bahasa Indonesia by a sworn translator except written in English; or
3. if the marriage is conducted in the Indonesian Territory: Marriage book or marriage certificate issued by an authorized ministry or institution
   
   

B. Spouse Visa:

1. Proof of guarantee from the Guarantor or a statement of commitment from the husband or wife
2. Marriage certificate or marriage book that has been translated into Indonesian by a sworn translator except those that have been written in English
3. Valid Limited Stay Permit/Permanent Stay Permit or Limited Stay Visa of the husband or wife
4. A translated marriage certificate and the valid visa or stay permit (ITAS/ITAP) of your Indonesian spouse.

C. Child of Indonesian Citizen:

1. Visa Application from Indonesian parent.
2. Family card of Indonesian parents.
3. Proof of birth of the Foreigner, such as:
4. Birth certificate issued by an authorized government agency; or
5. Proof of birth reporting at the Indonesian Mission or the authorized government agency in Indonesia, if Foreigners are born abroad.
6. Proof of marriage of parents, such as:
7. Proof of reporting or registration at the Indonesian Mission or the authorized government agency in Indonesia and a marriage certificate that has been translated into Indonesian by a sworn translator except written in English; or
8. Marriage book or marriage certificate issued by the authorized government agency in Indonesia, if the marriage is carried out in Indonesia.

D. Stepchild of Indonesian Citizen:

1. Visa Application from Indonesian parent.
2. Family card of Indonesian parents.
3. Proof of birth of the Foreigner, such as:
4. Birth certificate issued by an authorized government agency; or
5. Proof of birth reporting at the Indonesian Mission or the authorized government agency in Indonesia, if Foreigners are born abroad.
6. Proof of marriage of parents, such as:
7. Proof of reporting or registration at the Indonesian Mission or the authorized government agency in Indonesia and a marriage certificate that has been translated into Indonesian by a sworn translator except written in English; or
8. Marriage book or marriage certificate issued by the authorized government agency in Indonesia, if the marriage is carried out in Indonesia.

E. Child Visa:

1. Proof of guarantee from the Guarantor or statement of commitment of his/her parents
2. Marriage certificate or marriage book that has been translated into Indonesian by a sworn translator except those that have been written in English
3. Birth certificate that has been translated into Indonesian by a sworn translator, except it is written in English
4. Parents' valid Limited Stay Permit/Permanent Stay Permit or Limited Stay Visa

F. Lawfully Adopted Child of Indonesian Citizen:

1. Visa Application from Indonesian parent.
2. Indonesian court decisions that explain the status of the legal relations between Foreigners with Indonesian parents.
3. Family card of the Indonesian parent.
4. A court decision confirming the legal relationship and the Indonesian parent's family card.

G. Parent of Indonesian Citizen:

1. Visa Application from an Indonesian citizen (child of the foreigner).
2. Proof of the legal relationship between parents and children as shown by the child's family card.
3. One of the following documents:
4. A birth certificate issued by the authorized government agency in Indonesia;
5. A birth certificate that has been translated into Indonesian by a sworn translator except written in English, if the child was born abroad; or
   - A child stipulation certificate if the child is not a biological child or illegitimate child.

H. Parent Visa:

1. Proof of guarantee from the Guarantor or statement of commitment of his/her parents
2. Birth certificate that has been translated into Indonesian by a sworn translator, except it is written in English
3. The children's valid Limited Stay Permit/Permanent Stay Permit or Limited Stay Visa.
